Deep caries management is not just about removing decay quickly. The clinical challenge is removing what needs to be removed while preserving as much tooth structure as possible. CeraBur® is useful because it gives a different tactile feel from conventional metal burs and is designed for controlled soft dentine excavation. There are different philosophies regarding the removal of deep caries; however, the CeraBur can ensure that all soft caries are completely removed prior to restoration. The surgical length burs can provide optimal visualisation while doing this delicate procedure and can also be combined with caries indicator gel.  When the pulp tissue is exposed, we recommend the use of one-fil putty to create a biological framework for pulp and tissues to attach, an alkaline environment, a biocompatible material, to provide optimal conditions to allow the pulp to retain vitality.

Research note

The existing Toothsaver page notes that, in a study prepared by Queen Mary University of London, CeraBur® showed a service life three times longer than a round tungsten carbide bur.

As with all caries excavation, use clinical judgement, appropriate magnification, radiographic assessment and caries management principles when deciding how much dentine to remove.
